This practice is committed to providing comprehensive quality medical care to all members of this community. All patient consultations and medical records are strictly confidential.

APPOINTMENTS: Routine appointments may be made by phoning our office after 8.30 am Monday to Friday. If your problem is urgent, please advise our receptionist. Appointments are usually made at 15 minute intervals, but if you have a complex problem, more than one issue, a special medical report, pre employment examination or a procedure it is recommended that you make a `long consultation'.

AFTER HOURS AND WEEKENDS: Doctor is available 24 hours a day for urgent medical attention. It would be helpful if you could phone the practice prior to arrival.

TELEPHONE ADVICE: While every effort will be made for Doctor to talk to you, we aim to minimise interruptions to Doctor's consultations with other patients. We may need to suggest a more suitable time for you to call back. If your need is urgent, please advise our Receptionist.

HOME VISITS: This practice recognises that some patients are either too frail or too sick to attend the surgery. Please phone to arrange a home visit. Our receptionist will give you an approximate time for doctor's visit. Since Doctor is available 24 hours for all emergencies, it is generally not appropriate for him to travel out of town.

NEW PATIENTS: New patients will be asked to complete a "patient information" form before the first consultation. If we need access to your clinical history from your previous Doctor, you will be asked to sign another form authorising this transfer of your information. FEES: We bulk bill aged pensioners and children under 16 years We request that all other patients pay our fee in full at the time of consultation. Fees may be paid by cash or cheque. A schedule of fees is displayed at reception. Concession card holders must present their current card to receive our discounted fee. Patients should note that services such as ECGs, X-rays, sutures etc will incur an additional fee. If you have any difficulty with our fees, please discuss this with us so that suitable arrangements can be made.

SPECIAL INTERESTS: Apart from comprehensive medical care, Doctor's special interests are minor Surgery, Adolescent Health, Obstetrics and teaching Medical Students. Confinements are no longer provided for at the Jandowae Hospital but Doctor is happy to share-care with Doctors at larger centres.

REPEAT PRESCRIPTIONS: At the time your medication is prescribed you will be provided with enough repeats until your condition is reviewed.

WAITING TIMES: We aim to ensure that Doctor sees patients promptly. However patients need to understand that emergencies may mean waiting. Our Receptionist will advise if the wait is likely to be significant.

TEST RESULTS: At the time the sample is taken, Patients will be advised when to telephone for test results.

HOSPITALISATION: Patients requiring hospitalisation will be admitted to the Jandowae Hospital, but will be referred on if necessary.

REFERRALS: A consultation is required for all specialist referrals. A referral is a legal document which allows you to claim a specialist rebate. Ensure you have your referral before your specialist consultation, as it is illegal to backdate. Patients should be aware that there may be a gap payment for services provided by Specialists including Pathologists, Radiologists etc

X-RAYS: Most minor X-rays can be performed at the Jandowae Hospital upon request from the Doctor subject to availability of qualified staff. However, it may be necessary at times to have X-rays performed in either Dalby or Toowoomba.

MEDICAL CERTIFICATES: Certificates for sick leave are legal documents and will only be provided if your medical condition warrants leave.

VISITING SURGEON: A Surgeon consults regularly at this practice. Please ask for details.

MEDICAL STUDENTS: From time to time Medical Students are attached to this practice. Students will be present during consultations only subject to your permission. We will ask for your written agreement to the student's presence.

OUR DATABASE: We need to know if you have changed your address or phone number or if your Medicare Card has been updated. You must present your current concession card if you think you may be eligible for a discount to our usual fee.
